We are looking for an enthusiastic and motivated Band 6 Mental Health Practitioner to join our dynamic Community Mental Health Team. In this role, you'll provide high quality, compassionate care to adults with mental health conditions. You'll work collaboratively with a well established team to deliver personalised treatment plans, conduct comprehensive assessments and provide support to patients and carers as they navigate complex mental health challenges.

To work with clients experiencing mental health concerns, and their families, in the Adults’ Community Mental Health Team within East Cheshire. The post holder will hold a professional qualification and have relevant experience regarding adult mental health care.
The post holder will be responsible for:
o Undertaking assessments and coordinating the care of service users.
o To work with family members closely to maintain high standards of collaborative care.
o To actively contribute to the day to day operational functions of the team such as completing new assessments, covering office “duty” and working as a team to deliver the team’s goals.
o To develop and maintain positive working relationships with service users, their carers, General Practitioners, third sector partners, other significant professionals, and members of the multi-disciplinary team.
o Ensuring that clear channels of communication are established and maintained with clients, carers, GPs, Social Services and other partners.
o To promote and identify areas requiring development within the CMHT in the Cheshire East area and to take a lead role in the development of services in conjunction with the Clinical Leads, Team Operations Manager, Head of Clinical Services and other professionals.
