Our Wycombe Rapid Response and Intermediate Care teams are busy places to work and you will be using your skills to support a caseload of patients with both acute and chronic conditions enabling them to maximise their independence. The role has been developed to support the clinical case managers and you will be working alongside Physiotherapists, Occupational Therapists, Nurses and Paramedics as well as rehabilitation focused healthcare assistants.
