We are looking to recruit an experienced specialist pharmacist to take on a senior role within our Buckinghamshire Lipid Optimisation Programme. This specialist programme delivers a population-health approach to lipid management across Buckinghamshire. Using specialist population health tools, we have delivered appointments to over 1,500 patients within the first 12 months and offered medication changes to over 1,200 patients. This includes over 500 patients being commenced on injectable lipid lowering therapies within secondary care.This role will support delivery of the programme, along with providing expert advice, support and training to colleagues within primary care.
