There is an exciting opportunity for a band 6 Specialist Nurse within the Community Palliative Care Team.The Community Palliative Care Team is a dynamic multi-disciplinary team who provide specialist palliative care services to Oxfordshire and South Northamptonshire. This includes home visiting, outpatient and Attend Anywhere clinics and telephone advice. We provide a 7-day service, including a Rapid Intervention in Palliative and End of Life care project which aims to enable patients whose preference is to remain at home for the end of their life to do so. Weekend and extended hours working will be required for this post.
