The successful candidate will act as a named professional in the assessment, planning, implementation and evaluation of rehabilitation care for patients. This will be achieved by devising evidence based, individualised plans of care.Rotations available at RDaSH are across Neuro Rehabilitation, Stroke Rehabilitation, Specialist Falls Service, Home First Services (community and inpatient), Enhanced Care Home Services, Older Peoples Mental Health Services, Learning Disability Services amongst others.
