The Intermediate Care service operates across Hull and East Riding providing care for patients requiring rehabilitation either in a community bedded unit or own home. Working in an active therapy and rehabilitation unit will give you the opportunity to look after patient requiring level 4 rehabilitation, stroke rehabilitation and also patients receiving end of life care.The unit is covered 24 hours a day, 7 days a week. The role requires to work across Specialist nursing teams including the Home Oxygen Service and Heart Failure service within Hull and East Riding. We are looking for a Health Care Assistant, who is keen to learn new skills and support the Specialist nurses in the delivery of care and assessments to patients within their service.
