An exciting opportunity has become available to join our Inpatient Rehabilitation Multi-Disciplinary Team at Meadow Court, in the role of our CHC Clinical Nurse Assessor and Discharge Facilitator.We are seeking a motivated and forward thinking registered qualified nurse to support the flow of patients through our inpatient rehabilitation units, whilst ensuring all patients discharged onto the D2A nursing home pathway are assessed against the criteria for NHS Continuing Care.The post holder will be responsible for the effective coordination of patient discharges throughout the inpatient units across Meadow Court.The successful candidate will need to be able to demonstrate knowledge, skills and experience of person-centred assessments and discharge facilitation.
