We provide admission avoidance and urgent community response to individuals in crisis in their own homes. You will be part of a team which provides , effective short term reablement/rehabilitation programmes to patients in their own home following recent hospital discharge, clinical triage as part of the NEAT (Network Escalation Avoidance Team) referral pathway and respond clinically to admission avoidance and support patients recently discharged home, with referrals coming from community services, the ambulance service and front door services at the acute hospitals.
