With a population of over 39,000 patients you will support 6 GP practices, all rated “Good” by CQC. You will be a valued member of our multi-disciplinary team of Clinical Pharmacists and Pharmacy Technician, Paramedics, Physiotherapists, Health and Wellbeing Coaches, Social Prescribers, Care Coordinators, and Nurse Associate.
