Derbyshire County Council and Derbyshire Community Health Services NHS Trust have recently agreed to explore integration of a range of services that support people to remain at home when their needs change, but also to return home following a hospital admission by receiving rehabilitation and reablement support. Both organisations have been committed to working together for a number of years and we already deliver multi-professional assessments, are flexible in how we use our workforce and we share good practice on training and development.
