We are passionate team who offer respite and short breaks to children and young people (0-18 years) with complex disabilities and health needs. We also offer palliative care services to children and their families within our Forget-Me-Not suite. The successful candidate will also have the opportunity to work with the wider CCN and Special Schools nursing teams, to support them as required. We are able to offer a rotational post that currently covers Hull and East Riding Community Rehabilitation and Hull and East Riding Intermediate Care. Our Community Rehabilitation team work with patients who have a variety of physical and neurological conditions, with the overall aim of them being able to maintain or improve their functional ability after illness/injury or a deterioration in their condition.We work alongside Physiotherapists and Associate Practitioners to achieve patient goals, which can be as varied as being able to prepare a meal independently following a brain injury, to being able to get up the stairs to use the bathroom following a hip fracture.
