CARE NAVIGATOR25 Hours per week£12 an hourDue to the retirement of a long serving member of staff we are looking for a new Care Navigator to join our Patient Support Team.We are a busy but friendly Practice with a list size of around 5400 patients, situated in a purpose-built premises.Our team includes: 3 GP Partners, 1 Salaried GP, 2 Advanced Practice Nurses, 1 Practice Nurse, 1 Nursing Associate, 2 Healthcare Assistants, 1 GP assistant, Management and Patient Support Team.We are an established Training Practice for ST3s (GP Trainees), ST2s, Foundation Doctors and medical students (nurse, paramedics etc).Working hours are as follows although applicant must be flexible as per needs of the Practice: Monday 8am - 1pmTuesday 8am - 1pmThursday 8am - 6pmFriday 1:30pm - 6:30pmExperience is prefe...
