An exciting opportunity has arisen for a post with the Prevention and Early Intervention Services for an Adult Support Coordinator. The post will be based in the Adult Early Help Team. Adult Support Co-ordinators play a major part of the work we do in our Teams. Youll be working with adults and older people in need of care and support services, along with their informal carers. It will be crucial that you work in partnership with them in order to establish needs and ensure theyre met. The Adult Early Help Team is the front door to Adult Social Care and we look to provide advice, guidance and onward referrals to enable people to remain independent in their communities.
