Duties and responsibilities for the role include but are not limited to: Provide Mental Health assessment, routine and urgent, including risk assessment and formulation of the mental health and treatment needs for referred patients, acting as care coordinator. Provide recommendations for maintaining maximal employment, applying an occupational health matrix. Make decisions to determine if patients require further intervention from the mental health teams (DCMH/PWS), including admissions to inpatient services. Participate and advise in the pre-admission assessment pathway, facilitate patient pathway to DCMH to ensure continuity of care in the patient pathway.
