An exciting opportunity has arisen for a Senior Social Worker/ Approved Mental Health Professional - Grade 11 to join our community mental health Older Peoples integrated service based at Drury Lane Health and Wellbeing centre.The post is in Partnership with Wakefield Local authority and Southwest Yorkshire Partnership NHS Foundation Trust.You will work as part of a Multidisciplinary team, along side other professionals and support staff.The post holders will as part of your role, undertake assessment, including risk assessment, plan interventions, complete Care Act / conversation-style assessments, strength-based reviews, and develop person-centred, and outcome-focussed care and support plans.
