To work as a member of the Rapid Response & Intermediate Care Team in (the community) in Buckingham and surrounding villages providing nursing, social and rehabilitative care which may include some nursing, physiotherapy and occupational therapy-based interventions for designated patients (either individuals or a specific clinical group).
