Virtual Ward is coming to Norfolk Integrated Care System with its main aim of keeping patients at home during acute illness and therefore avoiding unnecessary hospital admission. This is a technological assisted approach with remote monitoring capabilities with a focus on patients with respiratory disease, frailty and heart failure. This is a face to face model (also referred to as 'hospital at home') therefore a multidisciplinary approach is required.
