This is an exciting opportunity for you to come and join our friendly team.We are a multi-agency team which provide health and wellbeing education and promotion to adults and older people in their own homes, over the telephone and via other routes i.e. group sessions and community events. We also help deliver the NHS Health Checks program in the community and at outreach venues across West Sussex.This will be a band 5 - 6 development post. This is a great role for a nurse with an enthusiasm for Health Promotion and a good knowledge of local health and social care. A band 5 nurse who wants to further develop their skills in early health intervention.
