We are looking for a Discharge Coordinator to join our Rapid Transfer Team based at QEQM in Margate. You will work as a key member of the Rapid Transfer Service, whose aim is to ensure the timely, effective, appropriate and safe discharge of patients requiring supportive discharge, from the acute into a community setting.
