JOB PROFILE The post holder will care for patients in the community and work within the Ambulatory Outreach Team (AOT). Assist in the development, implementation, delivery and evaluation of the AOT [Acute Hospital at Home (AHaH) & Complex Outpatient Parenteral Antimicrobial Therapy (COPAT)] function and contribute in further development and streamlining of the AOT. As a senior nurse, the post holder is expected to work and provide specialist input, and flexibly rotate within both services. The post holder is expected to work autonomously and support other team members.
